If you want to see if an individual you are working with has a Medicaid Waiver or is on the Waiver Waiting list (Developmental Disabilities) contact- Elise Washington 504 620 2204 (note that a consent form is needed in order for metropolitan to release this information)
If an individual you work with is receiving services but is in need of additional services (Mental Health- Adults) contact- Renata Stark 504 568 3130 ext 2603 or Mary Perrault-Simmons ext 2455 and fax attached form signed by the individual to 504 942 5910
Please only use this attached form for clients who have No Insurance, GNOCHC, or Medicare & QMB combination only. If a client has a Bayou Health Provider please refer the client directly to the agency for that Case Management/ACT/CPST/SA referral
